Being of lawful age, in consideration of being permitted to participate in "Turn Up & Play" sessions, I release and forever discharge Halifax Friendly FC, its owners, directors, officers, employees, agents and representatives from all claims for injury to person or property, except for death or personal injury caused by the club's negligence or breach of statutory duty, which cannot be excluded by law.

I acknowledge that participation entails a risk of physical injury (including collision, sprains, fractures and concussion), and that I am freely and voluntarily taking part. I confirm I have no physical limitations, medical ailments, or disabilities that would prevent safe participation, and will seek medical clearance if required.

The club does not provide personal accident or injury insurance for participants. If I want that protection, arranging it is my own responsibility and at my own cost.

I confirm all information I have provided is true and accurate. If it is false or misleading, I accept responsibility for any resulting loss or claim myself rather than the club.

This waiver is governed by the laws of England.

This consent applies to all "Turn Up & Play" sessions the participant attends, in their correct gender category, for the remainder of the current football season or until withdrawn in writing, whichever is sooner.

I understand and acknowledge that the FA strongly advises against children under 16 playing football with adults, and that there is an increased risk of injury (including collision, sprains, fractures and concussion) in doing so. I have been made aware of this and choose voluntarily to consent to participation.

As the participant is a minor, I agree that this release of liability and assumption of risk is made on their behalf and is binding on them so far as the law allows, and confirm I am the parent or legal guardian, with legal parental responsibility, and have full authority to agree to this on their behalf. I understand this cannot remove the participant's own separate right, once they turn 18, to claim in their own name for injury caused by the club's negligence.

The club does not provide personal accident or injury insurance for participants. If I want that protection, arranging it is my own responsibility and at my own cost.

I accept responsibility for any and all risks in the activity, except liability for death or personal injury caused by the club's negligence or breach of statutory duty, which cannot be excluded by law. I am responsible for the participant's safety and supervision (or that of my named substitute) at every session.

I confirm all information I have provided is true and accurate, including my relationship to the participant and my authority to sign. If it is false or misleading, I accept responsibility for any resulting loss or claim myself rather than the club.
